YOUR ASSISTANCE IN MAKING SURE YOU TURN IN A COMPLETE PACKET IS GREATLY APPRECIATED!!!!!

Each applicant must complete a single application packet, which will be considered for each scholarship that is indicated on the checklist.

PLEASE include as much detail in your answers as possible.

Applicants should be sure the packet contains:

• List of scholarships - check the specific scholarships for which you are applying

• A transcript from your high school or college

• Pages 1 - 4 vital information and answers to questions

• Additional letters of recommendation or essays as required for specific scholarships.

• A total of two Recommendations for Scholarships Administered or Awarded by the Southwest Iowa Educational Foundations (see attached form Page 5) should be completed and returned to you in sealed envelopes. Please submit them with the application, still in the sealed envelopes.

Completed packet is due in the counselor's office AS SOON AS POSSIBLE!

PLEASE PLACE YOUR MATERIALS IN THE FOLLOWING ORDER

• The List of Scholarships. (Mark the scholarships for which you are applying)

• Transcript from High School or College

• Vital Information and Answers to Questions Pages 1 - 4
  (COUNSELOR'S SIGNATURE on Page 1)

• Additional essays or information for specific Scholarships

• 2 Recommendations in Sealed Envelopes
