Application Instructions for
Jack Hyler Memorial Scholarship

Each applicant must complete a single application packet, which will be considered for each scholarship that is indicated on the checklist.

Applicants should be sure the packet contains:

• Pages 1 - 4 vital information and answers to questions

• A transcript from your high school or college

• A total of two Recommendations for Scholarships Administered or Awarded by the Southwest Iowa Educational Foundations (see Page 5 of Application From above) should be completed and returned to you in sealed envelopes. Please submit them with the application, still in the sealed envelopes.

Completed packet is due in the counselor's office on or before March 26, 2008

  COUNSELOR'S SIGNATURE on Page 1

• Transcript from High School

• 2 Recommendations in Sealed Envelopes
